PioneerCare receives grant to reduce meds [UPDATED]

The Minnesota Department of Human Services has awarded PioneerCare a two-year Performance Improvement Project grant to reduce medications and improve quality measures.sugar

PioneerCare is a member of the Care Ventures Cooperative, a collaborative of 16 rural nursing facilities in Minnesota. PioneerCare will work closely with other cooperative members and consultant geriatric pharmacists to develop and implement a specialty program aimed at the reduction of medications and treatments for residents at PioneerCare Center.

Staff will ensure all medications are relevant and necessary, and also identify ways to reduce psychopharmacologic drugs, with a focus on antipsychotics.

“Our ultimate goal with this grant is to reduce unnecessary medications and improve the quality of living for our residents,” said Nathan Johnson, Administrator for PioneerCare. “We are committed to clinical excellence and this grant is yet another example of how we are able to improve our delivery of care.”
